Fill up the form below, include obituary field details, upload  headshot photo of the deceased, and specify design options.

  • SUBMITTER INFO
    Name and contact information.
  • OBITUARY DETAILS
    Fill up the following fields: Name of deceased, age, cremation and memorial details, contact info, etc.
  • DECEASED PERSON'S HEADSHOT PHOTO
    This image will appear on the designated Obituary page as well as the Obituary Landing page.
  • FORMAT PREFERENCES
    Background colour, patterns, text colour, divider, etc.
  • PREVIEW OBITUARY
    You may go back and forth to Preview and make adjustments by toggling the Next and Previous buttons.
